Service Coordinator
Oakleaf Property Management is seeking to hire a full-time Service Coordinator. The Service Coordinator coordinates and links residents to needed services that will assist them to age in place, and provides information and makes appropriate referrals to residents and families in all areas of case management for services available in the community at large. This position is responsible for assisting Oakleaf Property Managementis residents (seniors and/or adults with disabilities) with the ability to thrive in their community. Service Coordinators achieve this by assisting residents to maintain their independence, with or without support of others. REQUIREMENTSMust have the ability to maintain professional boundaries, including exercising objectivity and confidentiality, while building trusting relationships with residents and staffPrevious case management experience helpfulExcellent written and oral communication requiredMust have excellent customer service, negotiation and communication skillsStrong computer skills, including ability to easily navigate the internet, use Outlook, create Word and Excel documents, flyers, newsletters and work with a web-based database.Must be familiar with community resources or have the ability to acquire those resources and become familiarMust be sensitive to the needs and concerns of residents and their familiesCommunity networking skills, knowledge of civic engagement and activities programming a plusMust have ability to complete a Service Coordinator Certification within the first year of employment, and 12 hours of continuing education every year thereafter.
